This is not just a triage card for disasters but a comprehensive documentation system for all people injured in civil or military emergencies. Unlike other systems which can only show a patient deterioration, the cruciform is flexible in use and allows for dynamic prioritisation. It is now recognised that patients can and will improve following trained immediate care. This system allows patients to be upgraded or downgraded in priority without difficulty or confusion

The card allows rescuers on scene and during transportation to provide the receiving medical staff with time based trauma scores, enabling definitive medical intervention earlier than might otherwise be the case. The receiving medical staff can of course use the system within the hospital environment, when patient volumes may be reducing the free flow of patients to, for example, X-ray department, theatres or wards.

Due to its carefully formulated contents, it also provides a better patient record for individuals or organisations faced with legal challenge and possible litigation after the event. For those patients who die, despite rescuer intervention, it enables the legal procedures to be initiated with a more comprehensive and continuous record of rescuers and care given than would otherwise be provided. It also enables additional information such as place/times and injuries found to be recorded.

All information recorded on the card is retained and there is no need to duplicate information if a priority is changed. This prevents loss of potentially vital data and saves rescuers time, something which is always in short supply during an emergency. The system has been designed by medical specialists trained and practiced in immediate care in conjunction with paramedic ambulance and other rescue staff. It has been evaluated under operational conditions by medics, both military and civilian in some of the most demanding environments in the world.

The card now also has a carbonated overlay sheet on the patient details page which can be given to the police, and a wrist band (numbered) plus number strip (adhesive and tear off numbered tags). The enables all rescuers records to be linked to casualty plus hospital records, property bags, infusion bottles etc. This will make post incident police/coroners officer investigation more accurate and will enable proper audit to take place.

Ideal for the contaminated casualty who can be stripped except for the wrists band and linked to his/her possessions after decontamination.

All the emergency services (and first aiders) have a legal obligation to HM Coroner to provide evidential continuity of the handling of a dead body from its location at the incident through its recovery to the post mortem examination. Victims will hopefully be alive when first seen, and treated/transported to hospital as soon as practicable but may, unfortunately, die shortly afterwards. It is necessary, therefore, for all first aiders and paramedics to consider the implications. They may be called upon to provide statements and continuity evidence about that victim at an inquest. The "Cruciform"© card will enable all emergency services personnel to provide that evidence.

  • The "Cruciform"© card is uncoated. Any pen will work on its surface and the writing on that card attested to in court at a later date. The original card (together with any other written documentation from any service involved) can be requested to assist the coroner in establishing the cause of death.
  • The numbered tags left at scene, wrist band on the victim and number stickers on medical/paramedic/police/fire brigade/hospital forms, bags of possessions etc., will provide a continuity chain from scene to post mortem and allow the rescuers to give accurate, sworn evidence about their involvement.

If only one person has handled the body then continuity is simple to establish. If several personnel, perhaps from different services, have handled the body then they must provide statements and continuity evidence about that body. The police, acting as coroner's officers, will gather all evidence and interview everyone involved with that victim to provide the continuity evidence required for the inquest. Individual rescuers can be called upon, in person, to give evidence about their involvement and attest to the entries and confirm the handwriting on documents.

Red card - Immediate
Yellow Card - Urgent
Green card - Delayed
White card - Dead

The delayed card can be folded to sub divide the group into two further sections, identifying, contaminated, and expectant patients

The card on the reverse has patient details and observations. These are timed as is the revised trauma score on the inside.

The card comes in a thick clear plastic cover and is attached to the patient by a long elastic tie. Each box of 10 or 25 cards also comes with a triage sieve/casualty estimate document and a pen.
